If we are out of network with your insurance company, we will be happy to provide you with a superbill or an itemized receipt after each visit that may allow you to get some reimbursement from your insurance company if you have out-of-network benefits.

- You will be contacted to gather more information if needed.
- After review, we will contact you to schedule an appointment or make appropriate referrals.
- Attend Your Session: Meet with our psychiatrist via our secure video platform.

| 1.First time Consultation- A comprehensive psychiatric consultation in an unrushed environment with the development of a personalized treatment plan in collaboration with you the patient and the parents [for youth]. This might take 1-2 Sessions. [60-90minutes] | |

| ii. Child [10-17] | $495 |
| 2. Follow-up appointments-Standard: 30-minutes | $150 |
| 3. Follow-up appointments-Extended: 45-60 minutes | $250 |
| 4. One time consultation: 40 minutes. This service is for prospective patients or professionals who are not sure they need services but would like a preliminary consultation prior to deciding. For patients, if you decide to schedule the appointment, $100 of this fee will be deducted from your initial consultation fee. | $150 |
